Every camper attending a Chrysalis weekend (caterpillars, team members, Outpost) is required to fill out a health form. Nobody will be allowed to attend Chrysalis without first completing the health form (this only applies to those spending the night at the camp during the Chrysalis weekend). If you have not received a Health Form or need another copy, please print one off and have it filled out by the time you arrive at East Bay Camp for the Chrysalis weekend.
