Anyone wanting to attend a Chrysalis Flight or Journey weekend must complete a registration form. Along with this registration form, a Sponsor Form must be filled out and submitted by the sponsor. Both forms are included in the following downloads.

Please Note: The registration deadline remains as the Saturday prior to the weekend. However, if we do not have enough candidates registered by the Thursday one week prior to the weekend, we may have to cancel the weekend. So please, get your registrations sent in a couple weeks prior to the weekend. Thank you!!
